Have you heard the term Social Prescribing being thrown around? No? See there’s a problem right there. Social Prescribing sets out to bridge the gap between health and wellbeing. It addresses the social determinants between the two. Many things that affect our health are non medical and social in nature, i.e. food security, financial, housing, social connectedness, transportation, etc.
At NSCR, we offer a program that supports seniors and/or their caregivers. Our Community Connector program offers one on one support to address some of the social needs and personal goals of an individual by asking the question: “what matters to you?”
